Rise and Root Behavioral Health Services, LLC is an outpatient behavioral health provider offering mental health and substance use disorder treatment services to adults and families, including individuals experiencing homelessness or housing instability. Services are delivered within a structured continuum of outpatient care and are designed to support stabilization, recovery, and functional improvement while individuals remain connected to community-based supports.

Services are provided by qualified and credentialed professionals using evidence-informed practices, individualized treatment planning, and coordinated care approaches. The organization prioritizes access to services for individuals with complex clinical and social needs, including those impacted by housing instability, and emphasizes ethical practice, interdisciplinary collaboration, and continuous quality improvement to support positive outcomes.

Rise and Root provides outpatient behavioral health services including individual therapy, group therapy, family and couples counseling, and behavioral interventions. Treatment planning is based on comprehensive assessment, client-identified goals, and medical necessity. Community-Based Rehabilitation Services (CBRS/PSR) are offered to support skill development, daily functioning, and community integration, particularly for individuals experiencing barriers related to homelessness or limited access to natural supports. Additional services, including medication management, case management, peer support, and family involvement, are incorporated when clinically appropriate and aligned with the individual treatment plan.

When clinically indicated, Rise and Root collaborates with community partners to support care coordination, housing transitions, and continuity of services, recognizing the impact of housing instability on behavioral health outcomes. Supportive services are integrated as part of a person-centered, recovery-oriented approach to care and are provided in accordance with Washington State Department of Health requirements and Medicaid (Apple Health) standards.
